Insurance Info & PPOs
- General Information
- Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage. The Oral & Facial Surgical Center makes every effort to provide you with the finest surgical care and the most convenient financial options. To accomplish this we work hand in hand with you to maximize your insurance reimbursement for covered procedures. Please bring your insurance information with you to the consultation so that we can expedite reimbursement.
- Payment in full is required for your office visit. For surgical procedures, one third (1/3) of the total amount plus any deductible, co-pay, or ineligible procedures will be due the date of surgery.
- We will submit your form to your insurance carrier for you. You are responsible for any deductible, co-payment, or ineligible procedures that your insurance refuses to pay.
- Most insurance companies will respond to your claim within four to six weeks. When your insurance has not responded to your claim within this time frame, we ask that you, as the policyholder, contact your insurance company. We have learned from experience that the insured gets a lot more done a lot faster with their insurance than we could ever do. If it is determined that additional information is needed from our office, we will be happy to provide it. If we have not received payment from your insurance company within 60 days, you will be responsible for your balance and, if necessary, re-filing of your insurance claim.
PPO Affiliations
Most insurance companies do not offer dental PPOs. We are PPO providers for the following Medical Plans: Baptist PPO and Healthlink
Insurance Patients - Please Read Carefully
The amount of coverage paid by your insurance company may be based on your insurance company's own reduced fee schedule for Oral and Maxillofacial Surgical services and may be less than actual charges resulting in lower coverage for you. We have no control over this situation. Lower payment is a direct result of the plan selected by your employer. Please be advised that we cannot waive co-payment. We are required by law to collect co-payment.
